For removal a slide hammer is the best tool, if you don't have a slide hammer you can drive the hub out of the knuckle from the inside out, using a socket or something the same size as the hub, if that makes sense.

The problem is that 16" 5x4.5 bolt pattern trailer wheels don't seem to exist, and even if they did they'd be dangerously rare if one of mine got messed up somewhere away from home. I resorted to using a pry bar to separate the hub and axle. In doing so I inadvertently pulled the axle out of its housing in the differential. With the extra space, I pulled the hub forward and worked parting the hub and axle using a wide-bladed chisel and a 3 lb sledge hammer. I rotated the axle and hub as I banged away heartily -- to no effect.

This is really old but doesn't appear to have been answered: my 2000 Echo takes a 12pt 30mm socket on the axle nut. It'll take over 200 lb-ft torque to loosen so do it before you raise the car, wheel still on the ground, E-brake on, jack in place ready to lift. The main trick is to push out the old bearings and press in the new ones without damage.
